Can we put this on a real survey platform maybe - asking everyone to copy/paste 17 free form questions in to here is going to be a mess!

Agreed, especially since flow discussions are not the typical communication style for many wikimedians. It would be easier to give feedback through a dedicated form
This should be possible, but it would entail a delay. I'll ask some folks and see if it could be worked out.
We're looking into it. Option A is to see if we can get an existing survey library into the app without too much work. If not, we'll re-work the entire thing some other way.
Hi @Xaosflux and @Wugapodes! Thanks for this very helpful feedback its much appreciated. We are awaiting approval from the legal department to launch the in-app form. Once we have the go ahead we will launch the mobile friendly survey form.
In the interim, we have created a button that auto creates a new section where you can respond to the questions. Please give it a try and let me know what you think.
