any idea why new entries are being duplicated in the database? the db has two rows for each entry i create through the wiki

i would if i knew how to reproduce the bug. it happened right on the first entry, after installing the extension. it made two entries in the database, with different id's.
then it happened again when i created the third or forth issues. seems random.
but it creates two DB entries with differnet id's, and when viewing the issue list, they both show up, as if they where two different issues.
can you reproduce the problem on the site http://wiki.epi-lyon.fr/index.php/Points_ouverts The user is utest / testtest

i don't remember doing anything strange apart from adding a new entry
but the db entries duplicated again ill send more info, maybe it'll help idk

I just installed IssueTracker 2.0 for my first time. I'm using MediaWiki 1.32.0. php 7.3.1. Any time I submit a new bug, it inserts it 3 times into the database. When I submit, httpd/access_log shows POST to index.php, then GET from index.php?title=<title>&bt_action=list. But, mysql/queries.log shows 3 separate INSERT's. Part of queries.log here: https://termbin.com/7p42 -- you will see there are other queries between. I haven't gotten it to add one without duplicating 3 times yet.
I just installed IssueTracker on MediaWiki. I had no problems adding new issues at first, but after issue #13 it enteres 3 times into the database any new issue. Any ideea how to fix this problem?
I have the same issue; it adds a duplicate entry for every insert. Is there any known fix?
I found a work around by specifying a unique project name when including the tracker on a page. It looks like having multiple trackers with the same project name, or using a blank project name was tripping me up.
