Install instructions for MW 1.25

Can someone please post a CLEAR, step-by-step, installation instructions for how to get this skin working with a MW 1.25?

I have tried doing it from the (short-hand) instructions posted below, but it just spits me out errors.

It won't install. The skin is buggy.
